Job Description
Under general supervision, provides comprehensive health services and supervises LPN staff if applicable, collaborating with providers to promote client wellness while maintaining confidentiality and ethical standards.
Conducts health assessments, identifies care needs, makes referrals, and assists clients with local resources.
Coordinates with staff, healthcare providers, and community organizations to ensure integrated client care.
Completes documentation related to assessments, monitors client health, administers injectable medications, and manages caseload records.
Supervises and delegates tasks to LPN staff, including medication monitoring and basic emergency interventions.
Builds team relationships, participates in staff development, hiring, scheduling, and evaluates program effectiveness.
Coordinates discharge planning, provides health education, and participates in quality assurance activities.
Acts in compliance with laws, regulations, and agency policies, supporting organizational mission and values.
Requires an Associate’s Degree in Nursing (Bachelor’s/Master’s preferred), valid NYS RN license, and at least two years of relevant experience; bilingual skills preferred.
Physical demands include sitting, walking, light lifting, and use of vision; work environment is typically quiet. Must possess a valid NYS driver’s license and reliable vehicle.
Must demonstrate excellent communication, organizational skills, confidentiality, and ability to work efficiently under pressure.
