Job Description

We are seeking a Home Based Primary Care Nurse Practitioner to serve as a primary care provider for elderly, homebound patients in Englewood.

  • Deliver comprehensive primary care, including acute, wound, hospice, and care management services at patients’ homes.
  • Assess social determinants of health, promote aging in place, and coordinate care to improve outcomes.
  • Utilize data and health information exchanges to monitor hospitalizations and ensure timely post-discharge follow-up.
  • Build strong relationships with patients, caregivers, and care teams through excellent communication.
  • Manage an empaneled patient panel, providing holistic, patient-centered care.
  • Participate in ongoing education, and adhere to schedules, typically Monday-Friday, with flexible hours and weekends off.
  • Support includes a sign-on bonus, competitive salary ($100K-$120K), benefits, auto allowance, PTO, CME allowance, and coverage for licensing and liability insurance.
  • Qualifications include a Master’s degree, Colorado licensure, with bilingual skills preferred.
