Job Description

Seeking a Primary Care Nurse Practitioner in San Antonio, TX, with a $5,000 sign-on bonus and relocation assistance.

  • Provide comprehensive at-home primary care visits to approximately 10 patients daily.
  • Manage health outcomes for an assigned patient panel, utilizing population health data to improve results.
  • Deliver condition-specific specialty visits as needed, maintaining strong communication with care team members.
  • Work Monday through Friday, with flexible schedules, rotating call (2-3 times/year), and weekends off.
