Job Description

We are seeking a Nurse Practitioner Float to provide primary care in North/Northeast San Antonio, covering home visits for 8-10 patients daily.

  • Deliver comprehensive, data-driven, condition-specific care at patients’ homes, including assisted living communities.
  • Manage health outcomes for an empaneled patient population and foster strong communication with care teams and external partners.
  • Utilize population health data to enhance patient results and participate in ongoing educational opportunities.
  • Qualifications include a Master’s degree and licensure in Colorado; bilingual skills are preferred.
  • Support staff includes MDs, triage, marketing, IT, billing, and scheduling.
