Job Description
This position seeks an Advanced Practice Provider to deliver behavioral health services in long-term care facilities.
Perform initial psychiatric evaluations and routine follow-ups for referred patients, monitoring medication efficacy and behavioral progress.
Document visits according to regulatory standards and coordinate care with mental health specialists when necessary.
Provide clinical insights for treatment interventions, oversee psychotropic reduction trials, and seek consultation as appropriate.
Collaborate effectively with team members, synthesize medical and psychiatric data, and manage special projects.
Ensure timely documentation for facility records.
Qualifications include a degree from an accredited Nurse Practitioner or Physician Assistant program, board certification in psychiatric mental health, and experience in behavioral health.
Skills required are knowledge of medical coding, familiarity with regulations (OSHA, HIPAA), proficiency in PC applications, leadership abilities, excellent communication, and a positive, solution-focused attitude.
