This Home Based Primary Care Nurse Practitioner role in Aurora offers a $5,000 sign-on bonus and relocation assistance.

  • It involves providing comprehensive primary care, care management, and on-demand services like acute diagnosis, wound care, and hospice at patients’ homes.
  • The position emphasizes holistic, longitudinal care, addressing social determinants of health, medication reconciliation, fall risk, and advanced care planning.

  • They will utilize population health data, coordinate with internal and external care teams, and see patients post-discharge within 72 hours.
