Job Description

We are seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team in a Skilled Nursing Facility setting.

This role involves serving as a clinical leader, collaborating with the care team to oversee residents’ short- and long-term needs. Responsibilities include admissions, discharges, stability checks, evaluations, advanced care planning, assessments, and prescribing treatments.
