Job Description

We are seeking a qualified LCSW for a part-time or full-time Geriatric Psych Social Worker role supporting long-term care facilities in Wisconsin.

  • Provide direct psychosocial services, including evaluations, diagnosis, and treatment for nursing home residents, with opportunities for individual, group, and family therapy.
  • Collaborate with an interdisciplinary team to ensure quality care and documentation in compliance with Medicare guidelines.
  • Manage a flexible caseload of 10-15 patients daily, with support from a dedicated practice manager.
  • Work daytime hours, enjoy autonomy within facilities, and utilize modern electronic health records.

    • Qualifications include an active LCSW license, 1-2 years of experience, strong communication skills, tech proficiency, and experience with healthcare settings preferred.
