Job Description

Seeking a Full-time Nurse Practitioner or Physician Assistant to deliver primary care in Skilled Nursing and Assisted Living Facilities in Portsmouth, NH, and nearby areas.

  • Provide primary care visits, medication reconciliations, and annual wellness exams 4-5 days a week.

  • Examine patients, review medical records, order diagnostic tests, diagnose conditions, prescribe medications, and develop treatment plans.

  • Educate patients and families on health conditions, prevention, and disease management.

  • Maintain accurate records, promote good health practices, and stay current with professional knowledge.
