Job Description

This full-time Physician role offers no call, no ER or hospital hours, excellent benefits, and student loan repayment through HRSA.

Position Summary: Responsible for overseeing APPs, clinical staff, and providing care to patients of all ages within a patient-centered medical home model. The role involves independent and team-based patient care, focusing 80% on managing acute/chronic illnesses, developing care plans, and coordinating referrals. It also includes mentoring APPs, developing workflows, and participating in quality improvement activities.

Key Responsibilities: 10% documentation, coordination of care, patient education, and team collaboration. The role emphasizes patient-centered, team-oriented care, with responsibilities in mentorship, meetings, and process improvement.

Qualifications: Current Colorado MD/DO license, board eligible/certified, DEA, BLS, valid driver’s license, and proficiency in office and EMR systems. Must demonstrate professionalism, confidentiality, effective communication, and teamwork.
