Job Description

Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.

Our team cares for a broad spectrum of patients, from short-term rehabilitation to complex geriatric cases, aiming to improve access, remove barriers, and deliver state-of-the-art care for optimal outcomes.

As a member of our team, you’ll participate in face-to-face primary care, educational sessions, and collaborate with Nurse Practitioners and Physician Assistants.
