Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to deliver equitable, value-based primary care for adults on Medicare in outpatient settings.

Provide independent, comprehensive care including routine and acute visits, wellness assessments, and health education.

Collaborate with a multidisciplinary team and coordinate with specialists and community resources.

Lead population health initiatives and support care outreach efforts.

Conduct home visits when needed and participate in promotional activities.

Qualifications: Master’s or DNP degree, national certification (e.g., Family, Adult-Gerontology, or Geriatric NP), active state license, and DEA registration.

Preferred: Experience in primary care, understanding of HCC, ICD-10 coding, and bilingual skills are a plus.
