Speech Language Pathologist (Home Health) (Part time/PRN)

Job Description

We have a Part-time/PRN opportunity for a licensed Speech Language Pathologist (SLP) to assess, diagnose, and treat communication and swallowing disorders in patients’ homes.

• Work with patients experiencing cognitive-communication and swallowing issues due to stroke, injury, illness, surgery, or dementia.
• Conduct initial assessments and develop individualized treatment plans.
• Collaborate with the care team to ensure continuous patient care.
• Record notes and maintain documentation in the health information system.
