Job Description

Seeking a dedicated Primary Geriatric Therapist to provide individual, group, and family therapy for adults in a nursing home setting within PHP/IOP programs.

• Develop, implement, and document personalized treatment plans in collaboration with an interdisciplinary team.
• Maintain compliance with state regulations and professional standards.
• Engage residents in therapeutic activities and foster a supportive environment.
• Complete accurate documentation in electronic health records.

Qualifications include a Master’s degree in a mental health field, active Texas licensure (e.g., LCSW, LPC), and preferably experience with geriatric populations or PHP/IOP settings.
