Seeking a PER DIEM Speech Therapist for a Skilled Nursing Facility in Rochester, NY, located at Wesley Gardens, 3 Upton Park.

Provide quality SLP services, including evaluations, screenings, referrals, data collection, and care planning.
Complete timely documentation such as progress notes, reports, and discharge summaries.
Expertise in short-term and long-term SLP care, with experience in cognitive and dysphagia rehabilitation.
Educate patients, families, and staff on goals, progress, and therapeutic techniques.
Ensure compliance with protocols and regulations.
Qualifications include a valid NYS License, CCC, and preferred knowledge of Rehab Optima. Must be able to work in person and relocate to Rochester, NY 14607 prior to starting.
