The Primary Care Physician plays a vital role in a multidisciplinary team delivering comprehensive, patient-centered healthcare in an urban community health center.

Responsibilities include managing a patient panel, reviewing labs and imaging, coordinating referrals and transitions of care, ensuring accessible and thorough documentation, and actively engaging patients in their care plans.

Qualifications:
– MD/DO with residency in Internal or Family Medicine, board eligible or certified;
– Bilingual Spanish preferred;
– Active MA license, DEA, BLS within 90 days.

Skills required include strong communication, organization, teamwork, and quality improvement participation. The role emphasizes professionalism, community engagement, efficiency, and adherence to policies, with a salary range of $175,000-$230,000 plus benefits.
