Job Description

The Palliative Care Advanced Practice Provider (APP) is a key member of the interdisciplinary team delivering goal-concordant care in long-term and skilled nursing facilities.

  • Conduct comprehensive assessments and manage symptoms such as pain, dyspnea, and distressing conditions.
  • Engage residents and families in goals of care and advance care planning, documenting discussions per guidelines.
  • Facilitate hospice referrals and end-of-life planning, supporting transitions and family needs.
  • Collaborate with healthcare teams, including social workers and nursing, to ensure coordinated care.
  • Provide education to residents, families, and staff on disease progression and comfort strategies.
  • Maintain thorough documentation, participate in quality improvement, and adhere to protocols.
  • Maintain licensure and pursue ongoing professional development.


  • Position is full-time, based in facilities, working four days a week with on-site duties and rotating remote call. Requirements include bedside evaluation skills and emotional resilience. Benefits include competitive salary, insurance, retirement plans, and paid time off.
