Job Description

We are seeking a compassionate and skilled Nurse Practitioner or Physician Assistant to join our outpatient diabetes clinic in Keene, NH.

Conduct patient visits, develop individualized care plans, and focus on disease prevention and glycemic control.

Collaborate with endocrinologists, educators, dietitians, and nursing staff to provide holistic care.

Educate patients and families on diabetes management, lifestyle changes, and medications.

Order and interpret labs, adjust treatments, and monitor outcomes.

Participate in quality improvement initiatives and support program growth.

Qualifications: Active NH licensure, national certification (PA, FNP, or AGNP), and prior experience in endocrinology or primary care. Strong clinical judgment and communication skills are essential.
