Job Description

Seeking a full-time Advanced Practice Provider (PA or NP) passionate about compassionate wound care and patient healing.

Responsibilities include providing advanced wound care in patients’ homes, conducting comprehensive assessments, developing individualized care plans, managing medications, and educating patients and staff.

Qualifications:

Graduate of an accredited nurse practitioner or physician assistant program

1+ year of clinical experience, wound care experience a plus

Board certification through AANP, ANCC, or NCCPA

Current/unrestricted license in Minnesota and ability to obtain DEA/licensure

Willingness to travel 75% of the time
