Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide high-quality, patient-centered primary care in skilled nursing and long-term care facilities in Minnesota.

Perform assessments, physical exams, and develop treatment plans for acute and chronic conditions.

Order diagnostics, prescribe medications, and educate patients, families, and staff.

Collaborate with care teams to ensure seamless, comprehensive care.

Qualifications include a Minnesota NP license, board certification, and preferred experience in geriatric or post-acute care.
