Seeking Board Certified Geriatricians, Internists, and Family Practitioners to join a dedicated team providing comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.
Responsibilities include delivering face-to-face primary care, participating in educational sessions, and collaborating with advanced practice providers. The role aims to enhance access, remove barriers, and deliver state-of-the-art care to improve clinical outcomes.
