Job Description

The LCSW-Social Worker for Case Management provides psychosocial support to patients with complex needs, collaborating with healthcare teams and community agencies to ensure safe discharge planning.

Interventions include assessment, referral, crisis intervention, and care management to help patients and families cope with emotional and biopsychosocial challenges affecting care.

Requires a Master’s Degree in Social Work, LCSW license, and current BLS certification. Prefer one year of supervised clinical experience in a medical setting.

Knowledge of psychodynamic and crisis theories, brief counseling, and community resources is essential.

This full-time role offers a sign-on bonus, hourly pay between $40.00-$48.10, and benefits including health, dental, vision insurance, paid time off, and flexible spending accounts.
