Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an independent APRN or Physician Assistant responsible for managing patients in PALTC settings to enhance care quality, patient experience, reduce hospitalizations, and lower costs.
Responsibilities include conducting assessments, diagnosing and treating illnesses, developing care plans, providing preventive services, and collaborating with facility staff and external providers. The AP works autonomously within scope, ensuring compliance with regulations, maintaining accurate documentation, and participating in quality initiatives.
They oversee infection control, educate staff and patients, and demonstrate strong clinical decision-making, communication, and leadership skills. Proficiency with EHR and telemedicine tools is essential. Qualifications include a relevant degree, 2+ years of experience, and board certification. The role is full-time, in-person within PALTC facilities, offering comprehensive benefits.
