Job Description
The Licensed Clinical Social Worker (LCSW) provides evidence-based psychotherapy, including individual, group, family, and couple counseling to adult and youth patients within a primary care team.
Reporting to the Chief Strategy Officer, the role involves conducting assessments, diagnoses, developing treatment and self-management plans, and offering crisis intervention and referrals. The LCSW collaborates closely with clinic staff, external partners, and medical providers to address behavioral health needs, promote health education, and improve patient outcomes.
Responsibilities include documenting sessions in the EHR, supporting quality improvement initiatives, participating in team meetings, and providing training to staff. The position requires cultural competence, adherence to policies, and confidentiality, with some travel within the region.
