Job Description
The Certified Nurse Practitioner in the Nursing Home serves as a key member of the Interdisciplinary Care Team, providing care management, clinical assessments, and education to residents, families, and staff.
They facilitate early diagnosis, intervention, and coordinate services across healthcare providers to ensure quality, cost-effective care.
Daily visits include assessing residents in 1-2 nursing homes, especially post-hospital discharge, conducting exams, prescribing treatments, and evaluating episodic complaints.
They educate beneficiaries and families on condition management and advocate for their needs.
Qualifications include a valid Ohio Nurse Practitioner license, at least 3 years of healthcare experience (preferably in HMO/PPO settings), and no licensing suspensions.
Work hours are Monday-Friday, 8am-4:30pm, with a great work/life balance.
