Job Description

Join a mobile primary care team providing compassionate healthcare to seniors in adult family homes, memory care, and assisted living communities within Salem, OR.

  • Offer patient-centered care managing chronic conditions like CHF, hypertension, and diabetes to improve quality of life.

  • Operate independently with strong support, managing patients within a 25-mile radius.

  • Enjoy a flexible Monday-Friday schedule, competitive pay ($160,000-$180,000), and comprehensive benefits including health, dental, vision, and PTO.

  • Must be a licensed NP/PA-C in Oregon with DEA and BLS/CPR certifications, reliable transportation, and a passion for geriatric care.
