Job Description

We are hiring an APP for our mobile geriatric practice focused on wellness and disease management in assisted living communities.

  • Join a supportive, team-oriented environment with full autonomy in primary care services.
  • Provide in-person care at assisted living facilities in Danville, Arcola, and Champaign, visiting one community daily.
  • Work alongside dedicated full-time LPNs across multiple communities, covering 2-4 facilities.
