Post Acute Nurse Practitioner/ Physician Assistant – Dover, NH

Job Description

Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Dover, NH, and nearby areas.

• Perform primary care visits, medication reconciliation, and annual wellness exams 3-4 days weekly.
• Examine patients, review medical records, order diagnostic tests, diagnose conditions, and propose treatments.
• Prescribe medications, maintain accurate documentation, and educate patients and families on health conditions and prevention.
• Oversee overall patient health and promote good health practices.
