We are seeking a compassionate Nurse Practitioner (NP) for a Skilled Nursing Facility in Johnstown, NY.
Provide comprehensive assessments, develop treatment plans, and manage acute and chronic conditions in collaboration with physicians. Monitor patient progress, order and interpret diagnostic tests, prescribe medications, and document in the EHR. Participate in team meetings, coordinate care with interdisciplinary staff, and ensure compliance with regulations. Require NY NP license, AGNP/FNP certification, DEA registration, and 1-2 years of geriatrics or long-term care experience.
