We are seeking a Licensed Therapist to join our Collaborative Care Team in an integrated primary care setting.
Under supervision, provide diagnostic assessments, brief interventions, care coordination, and psychoeducation to support patient mental health. Utilize measurement-based care tools to monitor progress and collaborate with medical and behavioral health professionals. Maintain accurate documentation in compliance with HIPAA and ethical standards. Qualifications include an active California license (LMFT, LCSW, PhD, PsyD), 2+ years experience preferred, with Spanish fluency and familiarity with evidence-based modalities being advantageous.
