Job Description

Seeking a Full-Time Hospice Social Worker to provide compassionate support to patients and families in Merrillville, IL, and surrounding areas.

Responsibilities include conducting psychosocial assessments, assisting with needed services, and traveling to patients’ residences or facilities.

Requirements:

Master’s of Social Work and valid social work license

Driver’s license, insurance, and vehicle

1-2 years healthcare experience preferred
