Job Description
Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.
Deliver face-to-face primary care services and participate in educational and outcomes review sessions. Collaborate with Nurse Practitioners and Physician Assistants. Work across a spectrum of patients from short-term rehab to complex geriatric cases.
