Job Description
We are seeking a full-time or part-time Nurse Practitioner (NP) to join our Primary Care team in Bethany Beach, DE.
Key responsibilities include:
Providing direct patient care and developing comprehensive care plans
Building trusting, long-term relationships with patients and families
Monitoring patient progress and coordinating care with healthcare teams
Communicating care plans and health education clearly
Documenting encounters accurately using ICD-10, CPT II, and HCC codes
Participating in team meetings and quality improvement initiatives
Qualifications and attributes:
Certified Registered Nurse Practitioner (CRNP) with active, unrestricted license
Commitment to high-quality, high-value care aligned with ACO principles
Self-motivated, proactive, with excellent communication skills
Collaborative, professional, and dedicated to ongoing learning
Ability to commute or relocate to Bethany Beach, DE prior to start
Work is in person, with a schedule of 24-40 hours per week.
