Seeking a full-time Home Care RN Clinical Manager to lead and supervise a home health care team, ensuring delivery of high-quality services in accordance with regulatory standards and agency objectives.
Responsibilities include overseeing team performance, managing patient care plans, ensuring documentation accuracy, and facilitating quality improvement initiatives.
Provides clinical oversight, reviews OASIS and EMR documentation, and coordinates patient schedules to optimize care outcomes.
Requires at least 2 years of experience in a licensed Medicare home health agency, Colorado RN licensure, and familiarity with federal and state regulations.
