Job Description
The Certified Nurse Practitioner in the Nursing Home serves as a key member of the Institutional Interdisciplinary Care Team, providing care management, clinical assessments, and education to members, families, and staff.
Responsibilities include triaging care needs using HRAT scores, conducting ongoing clinical assessments, coordinating with other practitioners for access to various services, evaluating patients post-discharge, addressing episodic complaints, and promoting self-management through education.
This full-time role offers a Monday-Friday schedule (8am-4:30pm) with a focus on work/life balance, typically visiting 1-2 nursing homes daily.
Requirements include a valid Ohio Nurse Practitioner license, no history of license suspension or Medicaid/Medicare exclusion, and at least 3 years of healthcare experience, preferably in an HMO/PPO setting.
Compensation ranges from $110,000 to $115,000 annually, with benefits like 401(k), insurance options, paid time off, tuition reimbursement, and flexible scheduling.
