Job Description
Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Bennington, VT, and surrounding areas.
Responsibilities include conducting patient exams, managing medical records, ordering diagnostics, diagnosing illnesses, prescribing medications, and educating patients and families.
Provide wellness visits, medication reconciliation, and oversee overall patient health 4-5 days a week.
Offer comprehensive care within residents’ homes or facilities, utilizing an online portal for patient updates.
