Job Description

Seeking a full-time licensed mental health therapist (LCSW, QMHP, LPC, PA-C, or NP-C) to deliver integrated care in a busy primary care setting.
Must have an NPI before starting; preferred candidates are established with Medicaid and Medicare.

The role includes conducting initial evaluations, individual, group, and family therapy, and chronic disease management.
Provider will collaborate with primary care and psychiatric teams, support crisis management, and facilitate warm hand-offs for counseling.
