Job Description

Full-time Nurse Practitioner position focused on hospice, palliative, and house call care in Idaho.

  • Collaborate with a multidisciplinary team to provide compassionate, comprehensive care for patients with advanced or terminal conditions.
  • Conduct home visits and care in facilities, emphasizing advanced care planning and symptom management to support aging in place.
  • Perform face-to-face visits for regulatory compliance and symptom management, ensuring continuity of care with the hospice team.

    Qualifications: Valid NP license for Idaho and Oregon, board-certified (ANCC or AANP), valid driver’s license, and willingness to obtain licensure in both states if only licensed in one. Travel up to 75% preferred. The role is in person, based in Meridian, ID, with a Monday–Friday schedule.
