Job Description

Seeking a licensed Nurse Practitioner passionate about senior care and innovation to join a team dedicated to helping seniors live safely, healthily, and independently in their communities.

  • Provide outcome-based, comprehensive healthcare within legal scope, collaborating with an interdisciplinary team and supervising physicians.
  • Perform health assessments, physical exams, and patient education; order and interpret tests; manage acute and chronic conditions.
  • Coordinate care, participate in quality improvement, and support operational tasks alongside clinic management.
  • Qualifications include PA licensure, experience with elderly populations, strong communication skills, and ability to work independently.
