Job Description
We are seeking an experienced Assessment Counselor (LPC, LMFT, LCSW) for a full-time, hybrid role in an integrative medical facility.
Manage daily diagnostic assessments for mental health and substance use disorder patients, including appropriate care placement and treatment planning. Coordinate with clinical teams to ensure compliance with state and accreditation standards, utilizing electronic records and technology. Conduct assessments, develop initial treatment plans, and facilitate therapeutic groups, including tele-health sessions. Work independently and collaboratively within a multidisciplinary team, emphasizing trauma-sensitive, holistic care. Qualifications include a Master’s degree, valid licensure, certification (or ability to obtain within 6 months), and 3+ years of relevant experience.
