Job Description

We are seeking an experienced Assessment Counselor (LPC, LMFT, LCSW) for a full-time, hybrid role in an integrative medical facility.

  • Manage daily diagnostic assessments for mental health and substance use disorder patients, including appropriate care placement and treatment planning.
  • Coordinate with clinical teams to ensure compliance with state and accreditation standards, utilizing electronic records and technology.
  • Conduct assessments, develop initial treatment plans, and facilitate therapeutic groups, including tele-health sessions.
  • Work independently and collaboratively within a multidisciplinary team, emphasizing trauma-sensitive, holistic care.
  • Qualifications include a Master’s degree, valid licensure, certification (or ability to obtain within 6 months), and 3+ years of relevant experience.
