Join our team as an Advanced Practice Provider (APP) dedicated to delivering compassionate, person-centered, home-based care.
Provide holistic medical assessments, diagnosis, treatment, and patient education in home and community settings. Collaborate with a multidisciplinary team including nurses, social workers, and specialists to improve health outcomes and safety. Maintain accurate documentation, develop personalized care plans, and ensure compliance with legal and safety standards. Build trusting relationships with patients and families, advocating for their needs and resources. Participate in quality improvement initiatives and organizational goals. Qualifications include a graduate degree in Nursing or Physician Assistant Studies, valid licensure, national certification, BLS, and current DEA registration. Strong communication, clinical judgment, and ability to work independently are essential.
