Join our team as an Advanced Practice Provider (APP) dedicated to delivering compassionate, person-centered, home-based care.

  • Provide holistic medical assessments, diagnosis, treatment, and patient education in home and community settings.
  • Collaborate with a multidisciplinary team including nurses, social workers, and specialists to improve health outcomes and safety.
  • Maintain accurate documentation, develop personalized care plans, and ensure compliance with legal and safety standards.
  • Build trusting relationships with patients and families, advocating for their needs and resources.
  • Participate in quality improvement initiatives and organizational goals.

    Qualifications include a graduate degree in Nursing or Physician Assistant Studies, valid licensure, national certification, BLS, and current DEA registration. Strong communication, clinical judgment, and ability to work independently are essential.
