We are a rapidly expanding provider delivering innovative medical care in long-term and post-acute settings across the U.S.

We seek a full-time on-site Nurse Practitioner or Physician Assistant to provide care in Skilled Nursing Facilities in Corsicana, Texas. No nights, weekends, holidays, or on-call required.

Responsibilities include conducting assessments, developing care plans, managing chronic and acute conditions, ordering diagnostics, prescribing treatments, educating residents and families, collaborating with healthcare teams, and maintaining accurate documentation.

Requirements include current Texas licensure, board certification, experience with geriatric care preferred, strong communication skills, and proficiency with EMR systems. Occasional travel within the region may be needed.
