Job Description

The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently to provide comprehensive medical care in long-term care facilities.

  • Responsibilities include conducting physical exams, diagnosing and treating illnesses, managing chronic conditions, and providing preventive care.
  • Work autonomously within scope, making clinical decisions and acting as the primary provider, while collaborating with facility leadership and interdisciplinary teams.
  • Ensure compliance with regulations, maintain accurate documentation, and participate in quality improvement and safety initiatives.
  • Educate staff and patients on health promotion, and utilize EHR and telemedicine tools effectively.
  • Qualifications include a relevant degree, 2+ years of experience (preferably in long-term care), and active APRN or PA licensure.
