Job Description

Seeking a full-time Advanced Practice Provider (NP or PA) passionate about compassionate, high-touch wound care delivered in patients’ homes.

Responsibilities include assessing, developing, and implementing individualized wound treatment plans, performing advanced procedures, managing medications, and coordinating care with healthcare teams.

Qualifications include a minimum of one year in wound care and advanced practice, certification through AANP, ANCC, or NCCPA, and licensure in Minnesota. Wound care experience is a plus. Must be willing to travel approximately 75% locally.
