Seeking an Advanced Practice Provider to deliver comprehensive outpatient primary care in a Family Medicine setting at a clinic in Prineville, OR.
Under supervision, provide age-inclusive evaluation, diagnosis, treatment, and documentation of patient encounters.Coordinate medical, behavioral health, pharmacy, nursing, and ancillary services within a Medical Home model to optimize outcomes.Create and monitor personalized treatment plans, referring to specialists as needed.Participate in a shared call rotation, including evenings, weekends, and holidays.Ensure quality care, maintain accurate documentation, and adhere to regulatory standards.Collaborate with multidisciplinary teams, support quality improvement initiatives, and uphold professionalism and confidentiality.Required qualifications include a Master’s in Physician Assistant Studies or Nursing, valid licensure, certification, and experience in outpatient Family Medicine. Preferred candidates have 2+ years of experience, knowledge of Medical Home models, and Epic EHR proficiency.
