Job Description

The Nursing Home Wound Care Advanced Practitioner (AP) is a skilled clinical professional, either a Nurse Practitioner (NP) or Physician Assistant (PA), with specialized wound management training.

They are responsible for

  • Assessing and managing wounds in long-term care residents, including prevention and treatment of various wound types.
  • Developing personalized care plans and performing advanced wound therapies.
  • Collaborating with interdisciplinary teams to ensure comprehensive care.
  • Providing staff education, promoting prevention strategies, and ensuring regulatory compliance.
  • Monitoring outcomes, participating in quality improvement, and consulting with residents and families.
  • Addressing infections and advocating for resident needs, with strong communication and technological skills.

  • Qualifications include a relevant degree, 2+ years of experience, board certification, and preferably specialty wound care certification.
