Job Description
We are seeking a Nurse Practitioner to provide equitable, value-based primary care for adult Medicare patients in outpatient settings.
Qualifications: Master’s degree in Nursing, national certification in family, adult, or gerontological primary care, active NP license, DEA license, and US work authorization.
Preferred: Experience in primary care, knowledge of HCC coding, ICD-10, and passion for team-based, population health models. Bilingual skills are a plus.
