Job Description

Seeking an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) with wound care experience for a full-time, mobile role providing compassionate, in-home wound treatments.

Responsibilities include assessing patients, developing care plans, performing advanced treatments, managing medications, and coordinating care with teams and families.

Qualifications:

Graduate from an accredited NP or PA program

At least 1 year of clinical experience, wound care preferred

Board certification and valid licensure in the state

Willingness to travel approximately 75% to patient homes
