Join a dedicated team of Nurse Practitioners and Physician Assistants providing exceptional post-acute patient care in community facilities. This role involves managing and coordinating patient care, conducting rounds, documenting visits, and collaborating with healthcare teams. Comprehensive training, shadowing, and oversight by a Medical Director ensure you are well-supported.

Work 7 days on, 7 days off with 8-hour shifts, totaling 26 weeks off annually. We value compassion, communication, collaboration, humility, and a fun, team-oriented atmosphere.

Requirements include excellent communication skills, empathy, self-motivation, organizational ability, and relevant licensure (NP and DEA). Prior experience in geriatrics, internal medicine, ICU, or hospital settings is preferred.
