Nurse Practitioner

Job Description

We are offering a full-time Nurse Practitioner position within a Program of All-Inclusive Care for the Elderly, serving individuals 55+ with chronic illnesses or disabilities to promote independence in their homes and community.

Under supervision, provide comprehensive assessments, develop care plans, and manage acute and chronic conditions.

Coordinate with an interdisciplinary team to ensure holistic, personalized care.

Conduct in-person visits, reassessments, and manage hospitalizations and specialist referrals.

Participate in team meetings, quality improvement initiatives, and support policy development.

Maintain licensure, adhere to safety and confidentiality policies, and support patient rights.
